How To Apply

• Families must be referred by a medical or social service professional by email or mail.  Applications submitted directly by the family will not be approved.
• Families must have full legal custody of a child who is 18 years old or younger and has special health needs.  For childcare purposes, the child must not be dependent on a tracheostomy tube or ventilator.  The child also must not have any severe behavioral problems or highly contagious diseases.
• Heads of household must be highly motivated to obtain maximum self-sufficiency through employment, financial literacy, and adherence to the goals of their individual service plan.
• Family members over the age of 15 must be drug-free and must not have any felony convictions on their criminal history.
